There are a variety of ways it's distributed on onion sites. Imageboards, forums that require logging in, tube sites like pornhub, DDL filehosts, chatrooms, and just about every other way files can be transferred through the internet. Some free, some paid with crypto or even earning credits by uploading OC CP of your daughter/niece, and tons of scam sites that probably just take your money and give you nothing. A lot of it is uploaded to clearweb filehosts with links shared on tor. I've heard i2p and freenet have a lot of torrents of it too, and that people in countries that don't enforce CP laws just torrent it on their bare IPs.
